Applying to a service that needs a vision assessment? For police, firefighter and paramedic applications we do the whole thing in one visit — comprehensive eye exam, visual field test, and your form completed and signed by the optometrist.
Application vision testing — the eye exam, the visual field test and the completed form — is usually charged piece by piece, and typically starts around $200 and climbs from there. We keep ours deliberately affordable: one flat $175 for police, firefighter, paramedic and RCMP applicants.

$175 assumes the $90 exam is not OHIP-covered. If your eye exam is covered by OHIP — most commonly for patients 65+ or under 20, or with a qualifying medical condition — the exam fee doesn't apply, so you'd pay only for the visual field test and form signing. Bring the form your service gave you. Email us a copy too if you like, and we'll keep it on file as a backup in case you ever misplace the original.
Most vision assessments are valid for two years, so one visit usually covers you through the application process.
Most services ask you to submit your appointment receipt/invoice with the form. We give you both — and keep a copy on your file.

Driver's licence and MTO medical vision forms use a different, more detailed visual field test than employment applications. Because of that, they work a little differently from our $175 application testing — please read the three points below before booking.
The MTO visual field standard requires a more extensive test of your peripheral vision — it takes more time and concentration than the standard application test.
Because of the length and setup involved, the MTO visual field generally can't be done on the same day as your exam — we'll book it as a dedicated appointment.
Driving and MTO forms use our standard visual field fee of $120. The reduced $50 rate is a special discount we offer police, fire, paramedic and RCMP applicants only.
If your eye exam is covered by OHIP — for example if you're 65 or older, under 20, or have a qualifying medical condition — the $90 exam fee doesn't apply and your total is $120.
